¿Por qué puede ser difícil comprender qué es la TO? Una propuesta teórica desde cuatro perspectivas / Why could be difficult understand what is occupational therapy? A theorical proposal from four perspectives

El presente artículo es un ensayo desarrollado por cuatro terapeutas ocupacionales que en la actualidad desempeñan sus labores en dos universidades chilenas. Durante una mesa redonda realizada en el III Encuentro de Estudiantes de Terapia Ocupacional en Chile en el año 2014, y desde sus propios desarrollos teóricos, se reúnen para asumir el desafío de explicar por qué es tan difícil definir qué es la Terapia Ocupacional. Así, la discusión comienza con las propuestas realizadas por las mujeres pragmatistas, de principios del siglo XX, y los aportes para comprender los procesos de salud y enfermedad desde una perspectiva holística en contraposición a la visión dicotómica y reduccionista instalada por la ciencia neopositivista. Se prosigue con la propuesta ontológica de Heidegger sobre el “Ser y Tiempo” para la explicación de la ocupación y la Terapia Ocupacional, asumiendo un vacío ontológico en la historia de la profesión. Posteriormente se presentan los aportes que, desde la Teoría del Actor-red, ayudarían a responder la pregunta inicial, asumiendo que la dificultad estaría en la incapacidad de traducir y enrolar los propios desarrollos teóricos a otros actores. Finalmente se entrega una propuesta de posicionamiento epistemológico desarrollado inicialmente por Humberto Maturana y Francisco Varela, el que deriva hasta el paradigma de la complejidad, definido por Edgar Morin.

This article is the result of an essay developed by four occupational therapists that currently perform as academic at two Chilean universities. During a round table discussion, at the Third Chilean Meeting of Student Occupational Therapy in 2014, and from their own theoretical developments, they meet to take the challenge of explaining why it is so difficult to define what occupational therapy is. Thus, the discussion begins with the pragmatists women’s theorist of the early twentieth century and her contributions to understand the processes of health and disease from a holistic perspective as opposed to the dichotomous and reductionist point of view installed by neopositivism science. It continues with Heidegger’s ontological proposal of ‘Being and Time’ for explanation of occupation and occupational therapy, assuming an ontological gap in the history of the profession. Then, it presents the contributions from the Actor-Network Theory and how helps to answer the initial question, assuming that the difficulty would be the inability to translate and enroll themselves theoretical developments to other actors. Finally a proposal of epistemological positioning initially developed by Humberto Maturana and Francisco Varela, which derives to the paradigm of complexity, defined by Edgar Morin, is handed in.

Rol de los terapeutas ocupacionales en el trabajo con personas en situación de calle en Chile: aportes y dificultades desde la disciplina: una revisión bibliográfica / Role of occupational therapist working with homeless in Chile: contributions and challenges from the discipline: a literature review

La presente investigación tiene por objetivo establecer el rol de los Terapeutas Ocupacionales (T.O) dentro de políticas y programas de carácter público y privado, destinados a personas en situación de calle (PsC) en Chile. Este estudio cualitativo se basa en el enfoque de la Teoría Fundamentada, utilizando como estrategia de investigación, el análisis de artículos desde la disciplina que aborda el trabajo de la T.O con PsC en el extranjero, a través una revisión bibliográfica. Como resultados se obtuvieron siete categorías emergentes: comprensión del fenómeno, las causas que provocan esta realidad, el impacto de la intervención basada en la ocupación, los modelos teóricos utilizados desde la disciplina que respaldan la intervención, las capacidades, herramientas, funciones y dificultades de los Terapeutas Ocupacionales en el trabajo con esta población. Finalmente, se concluye que el rol del Terapeuta Ocupacional se orienta al desarrollo de habilidades necesarias para estos individuos considerando sus motivaciones e intereses, favoreciendo su inclusión social. Esto se relaciona con los valores y principios de igualdad resguardados en la Constitución de Chile (Ley 20.595) y con la visión de las entidades que trabajan con esta población.

The current research has the purpose of stablishing the role of Occupational Therapist (O.T) in policies and programs, both governmental and non-governmental, destined to Homeless in Chile. It was done through a qualitative view using a Grounded Theory methodology, through out a bibliographical review of articles from the discipline in regard with homeless and O.T issues. The findings arisen the following emerging categories: comprehension of the phenomenon, the causes of this situation, and the impact of interventions based on occupation, the theoretical models used from the discipline that supports the intervention, the skills, tools and purposes of O.T. and difficulties of O.T. working with this population. Finally, it is concluded that the role of the O.T. with this group of people is oriented towards the development of abilities needed to them effective tools for social reinsertion, based on the needs, motivation and interests considering their interests and motivations promoting social inclusion. These actions are relationship with the values and principles of equity guaranteed by the Political Constitution of Chile (Law N° 20.595) and the institutions working with this people.

La filosofía de la ocupación humana y el paradigma social de la ocupación: algunas reflexiones y propuestas sobre epistemologías actuales en terapia ocupacional y ciencias de la ocupación / The philosophy of human occupation and the social paradigm of the occupation: some reflections and suggestions on current epistemologies in occupational therapy and occupational science

Este ensayo corresponde al establecimiento de un posicionamiento epistemológico dentro del paradigma actual de la Terapia Ocupacional, el que hemos denominado Paradigma Social de la Ocupación. Dirigimos esta mirada desde la práctica profesional y desde el análisis crítico del conocimiento y posicionamiento científico de la Terapia Ocupacional y las Ciencias de la Ocupación. Dicho análisis se encuentra bajo una perspectiva histórica epistemológica (desde las raíces de la disciplina) y contextual (coherente con la cultura y la región), apoyada en la Filosofía de la Ciencia.La propuesta se desarrolla a partir de la revisión de los diversos y sucesivos posicionamientos epistémicos de la Filosofía de la Ciencia, desde los orígenes de la disciplina hasta nuestros días, realizando un meta-análisis, sobre la influencia de éstos en la conformación y desarrollo de la profesión. Para lo anterior, nos situamos en una rama de la filosofía de la ciencia, la que nos permite una reflexión de segundo orden sobre nuestra disciplina que hemos llamado Filosofía de la Ocupación Humana. Como primer ejercicio meta analítico, realizamos un análisis de segundo orden desde la Filosofía de la Ocupación Humana, en relación con la categorización hecha por Kielhofner sobre la práctica de la terapia ocupacional, según la cual se describen tres paradigmas que caracterizan a la Terapia Ocupacional en periodos históricos diferentes, influenciando la práctica clínica, los valores, los objetivos y la evidencia científica que han sustentado el quehacer profesional. Siguiendo esta línea, aparece un cuarto paradigma-Paradigma Social de la Ocupación-, el que se configura luego que la perspectiva única de la ocupación como fenómeno individual (tercer paradigma) no fuese concordante con las necesidades de las sociedades ni comunidades, ni respondiese de manera completa a los nuevos campos de acción de los terapeutas ocupacionales. This essay corresponds to the establishment of an epistemological position inside the current paradigm of the occupational therapy, which we call Social Paradigm of Occupation. We are making this proposal from professional practice and critical analysis of the scientific knowledge and position of the occupational therapy and the occupational science. This analysis is located in a historical, epistemological (from the roots of the discipline) and contextual perspective (consistent with the culture and the region) supported by the Philosophy of Science. The proposal is developed after reviewing the various successive epistemological positions of the Philosophy of Science, from the beginnings of the discipline until our time, doing a meta-analysis about the influence of these positions in the formation and development of the profession. To that end, we position ourselves in a particular branch of the Philosophy of Science that allow us to do a second order reflection from the Human Occupation Philosophy, regarding the categorization elaborated by Kielhofner about the practice of occupational therapy. This proposal describes three paradigms that characterize the Occupational Therapy in three different historical periods, which have been influencing clinical practice, values, objectives and the scientific evidence that have supported the professional work. Following this line, we conclude that it is necessary to the discipline to adopt a complex epistemology and thinking, according to the definition proposed by Edgar Morin. This way it would be possible to continue the theoretic development of the profession that allows us to answer to the new challenges that rise from the subjects we work with and their contexts.

Haciendo camino al andar: construcción y comprensión de la ocupación para la investigación y práctica de la Terapia Ocupacional / Understanding occupation for the research and practice of occupational therapy

El presente trabajo tiene por propósito sistematizar el proceso de reflexión, análisis y síntesis que hemos llevado a cabo en relación al desarrollo científico y académico de la Terapia Ocupacional. Da cuenta de los problemas que nos plateamos a partir de la creación de una nueva Escuela de Terapia Ocupacional en la Universidad Austral de Chile y propone una nueva forma para la comprensión de la Ocupación como objeto de estudio de la disciplina académica que sustente la práctica de la Terapia Ocupacional. Esta propuesta se presenta como el resultado de la síntesis de diferentes elementos propuestos por otros autores, en el marco de la tradición de la disciplina, así también como un punto de partida para la construcción de una ciencia que a la vez rescate los aportes de otros y permita la creación propia.

The purpose of the following article is to systematize the process of reflexion, analysis, and synthesis that we've lead regarding the scientific and academic development of Occupational Therapy. This work summarizes the issues which surfaced while creating a new Occupational Therapy program at Universidad Austral de Chile. It also proposes a new way of understanding Occupation, as the object of the academic research which sustains the practice of Occupational Therapy. The authors present this proposal as the result of the synthesis of different elements proposed by other authors in the framework of discipline's tradition, as well as a starting point for the development of a science which permits both taking knowledge from others and creating its own.
